Please fill this out when if we request it, or if you are interested in adopting one of our currently available critters needing a great forever home.  Please allow us up to 72 hours for a reply, although we usually reply much sooner than that!  We will respond to your application as soon as we can, via your preferred contact method that you mention in the form below – thank you!

Clinton Critter Care Adoption Application

In order to be considered to adopt a pet, you must:

• Be 18 years of age or older;

• If you are below 18 years, a parent or legal guardian should fill in the form;

• Have the knowledge and consent of your landlord if you currently rent;

• Have identification (of the same person who is filling up the form) showing your present address or verification purposes only;

• Be able and willing to provide a loving, safe and peaceful environment for the pet;

• Be able and willing to spend the time and money necessary to provide training, medical treatment and proper care for the pet.

<b>ELECTRONIC SIGNATURE REQUIRED: </b> **By typing your full name and today's date into the box below, you agree that if we approve your adoption application you are legally and morally agreeing to provide adequate and nutritious food and a constant supply of fresh water, as well as a clean enclosure, and you also agree to provide any and all veterinary, social, and health care for the animal(s) you are adopting at all times. <br><br> You further agree that you will never purposefully or by neglect cause harm or physical, mental or emotional suffering to this animal, and will give it the attention and love it needs and that this animal will never be a food item for another animal (if applicable). Clinton Critter Care will always re-accept animals which have been adopted, if for any reason the animal(s) does not work out in your home, or if you are unable to care for the animal(s) any longer for any reason, you agree to immediately contact Clinton Critter Care and surrender the adopted animal(s) back to Clinton Critter Care.<br><br> Any applicable adoption fees and deposits are non-refundable and go directly toward the care and keeping of our rescued animals. Finally, you agree that by signing and submitting this form, it DOES NOT GUARANTEE that you will be able to adopt the animal, and we don't necessarily go by the first application submitted, but the most ideal for the animal if there are more than one application, and we will also potentially check any references and information listed to determine if the animal you wish to adopt would be a good fit for your home.
